In consideration of students' safety, health, behavior, and to create a healthy and comfortable learning environment, our school requires every incoming freshman to undergo a physical examination and provide health information. This enables us to better address students' safety and health concerns.
1. Physical Examination (Mandatory)
Please refer to the attached DHSZ Student Physical Examination Requirements and complete the necessary examination items.
2. Student Health Information and Authorization Form (Mandatory)
Please complete the Student Health Information and Authorization Form. You can either fill it out before printing or print it out first and then fill it in. When providing a copy of the student's vaccination records, if your child has received the COVID-19 vaccine, please also submit a copy of the COVID-19 vaccination certificate.
3. Student Self-Medication Application Form] and [Medication Authorization Form (As Needed)
Students are not allowed to self-administer any medication on campus without school approval. In school, students can only self-administer medication after being assessed by medical service personnel and the school nurse. If boarding students bring any medication with them when returning to school on Sundays, they must immediately inform the dormitory supervisor of the name and quantity of the medication brought to school.
If your child needs to self-administer medication during school hours, parents must notify the school and submit the "Student Self-Medication Application Form." If approved, the student is only allowed to store a one-week supply of medication in the dormitory. Any excess medication beyond one week will be kept by the nurse or dormitory supervisor.
If you need the school nurse or a trained teacher to administer your child's medication, please fill out the "Medication Authorization Form.
