Medical Forms

To better protecting our students' safety and health, and to create a healthy and comfortable learning environment, our school requires every incoming freshman to undergo a physical examination in the hospital and parents need to complete school medical forms. This enables us to better address students' safety and health concerns.

Please submit the original (or a copy) of your child's medical examination report and completed school medical forms to school on School Dragon Induction Day, August 1, 2025, or on the first day of school.

1. Physical Examination -completed by a doctor in the hospital

Please refer to the attached DHSZ Student Physical Examination Requirements and take your child to the hospital to do the examination items required by our school.

You can either fill it out before printing or print it out first and then fill it in the above 3 forms. When providing a copy of the student's vaccination records, if your child has received the COVID-19 vaccine, please also submit a copy of the COVID-19 vaccination certificate.

3. Student Individual Medication Permission Form-completed by parents

(When your child needs to take the individual medication during school hours ）

The parent needs to submit a medical diagnosis record and the attached [Individual Medication Permission Form] to our school medical center if a student needs to take any individual prescription medication during school hours. All students’ prescription medication should be stored and administered in our medical center.
