Medical Forms

To better protect students' safety and health, and to create a healthy and comfortable learning environment, our school requires all new students to undergo a medical examination and complete the medical information form. This helps us better maintain students' safety and well-being.

Please submit your child's original (or a copy of) medical examination report and the completed medical forms to the school on July 31, 2026, or the first day of school, August 11, 2026.

1. Medical Examination (Mandatory)-To be completed by a doctor

     Please refer to the attached Medical Examination Requirements. Please take your child to a hospital to complete the required medical examination items.

2. School Medical Forms - To Be Completed by Parents (Mandatory)

You may choose to fill out these three forms online before printing, or print them out first and then fill them in manually

If your child needs to take any personal prescription medications during school hours, parents must submit the medical diagnosis records along with the [Student Individual Medication Authorization Form] to the school medical center. All students' prescription medications must be stored at the school medical center and will be managed and administered by the medical center staff.
